With Click4Care 360° Care Management™, the depth of our application is complemented
by a myriad of Channels to deliver the right information at the right time.
Within our multi-channel platform of telephonic/ IVR, Fax, Letter, Email and Web
Portal applications, Click4Care also provides preferred contact management,
ensuring all communication is provided via the modality of choice.
Our care management correspondence, documentation and letter generation functionality
is imbedded within Click4Care Foundation, enabling a streamlined approach for the
creation of accurate, comprehensive internally- and externally-oriented documentation
with template-based forms and letters, all integrated with configurable Provider
and Member Portals, allowing everyone within the care continuum to collaborate on
the Member’s plan of care in real-time. We have successfully deployed the application
for current customers to drive letters to local printers, in-house mail rooms, fulfillment
centers, and pick-and-pack vendors. We have also successfully integrated with external
letter frameworks.
Our Click4Care Channels capability was designed to facilitate and manage multiple
modes of correspondence. With a single unique letter template, correspondence can
be a mass communication targeting a specified population or a one-off ad-hoc letter.
Click4Care enables the process through flexible configuration; customizable letter
rules to member, employer, or product; and automated workflow and routing rules.
In addition to generating outbound correspondence in the form of letters, automated
workflow and routing capabilities allow our clients to effectively distribute work
internally, call reminders, health risk assessments, educational information, or
other program materials.
These integrated communications mediums are augmented byClick4Care’s configurable
Provider and Member Portals, allowing everyone within the care continuum to collaborate
on the Member’s plan of care. Our Provider Portal allows for Providers to automatically
and securely:
- Automate response to Provider authorization
requests that enter and track member data, reducing the need for interaction with
the UM call center resulting in less phone calls, less errors, time saved.
- Track the status of all of authorizations, not just those submitted through the
portal.
- Add an extension or appeal, without contacting the UM call center.
- Communicate via bi-directional messaging features with your organization to support
collaboration about members' plans of care and authorization details.
- View letters associated with an authorization.
- View metrics about member participation in case or disease management programs.
Click4Care’s Member Portal makes interacting with your membership a reality, as
we provide members to access and interact in real-time with his/her member’s plan
of care including:
- Workflow items (plan of care actions).
- Assessment activities.
- Personal profile editing (including managing their medications list).
- Authorizations (including participating in bidirectional communication regarding
authorizations).
