Click4Care Foundation is our core system, and our award winning capabilities include:
- Identification, Stratification and Analytics
native to our system. Our imbedded stratification offers instant, seamless management
of at-risk populations, gaps in care identification, and building of personalized
care plans, providing your resources the advantage of automated interventions that
are personalized for your members.
- Integration - We have successfully integrated our solution with a variety of core
administration/claims adjudication systems and have out of the box interfaces built
to accommodate InterQual and Milliman clinical criteria. Our powerful integration
pulls information from all appropriate external applications (claims, pharmacy,
labs, clinical decision support tools, clinical criteria systems, etc.) into the
Member Record, enabling your staff to interact with only one system. This access
to external systems will unlock the power of your information across data sources
and enhance your ability to collaborate between care managers, members, providers
and social support resources.
- Integrated Workflows for all care management activities - Click4Care provides users
with a unique Work Board – a virtual “Mission Control” where all CM, DM and UM notices,
alerts and overdue items are highlighted to bring the most important aspects of
the workload quickly and easily to the user’s attention. Additionally, a supplemental
Supervisor Dependent View (SDV) is available through the User Interface, allowing
care managers and supervisors a quick view into their care team’s progress.
- Health and Wellness is included within the Click4Care 360° Member Record and integrated
with Case Management, Disease Management, and Utilization Management to continually
keep all areas of the Member Record updated. Our comprehensive and integrated Care
Management capabilities are uniquely designed to have best-in-class workflow configuration
and care plan functionality designed by our product management team made up of experienced
clinicians. The result for our customers is a positive effect on MLR while also
fully automating the administration of wellness programs that not only save on members’
claim costs, but also promote healthy behaviors.
- Health Risk Assessments - The Click4Care Member Record can consume external HRA’s
or the client can configure a HRA within the solution utilizing Click4Care's full
suite of content authoring and workflow management tools, including risk stratification
tools. Additionally Click4Care can support the calculation and tracking of Incentive
Based Benefits.
- Click4Care Rules Engine, a seamless and transparent coordination among Utilization
Management, Case Management, Disease Management and Health and Wellness functionality,
simultaneously providing real-time updates, alerts, and Gaps in Care Messages, to
the member record as well as the user’s Work Board.
- Channels - Our communication can be by mail or e- fax; letter, email and web portal
applications. We have also successfully deployed our application to drive letters
to local printers, in-house mail rooms, fulfillment centers, and even pick-and-pack
vendors. We have also successfully integrated with external letter frameworks. Click4Care’s
multi-channel platform provides preferred contact management across telephonic,
IVR, Fax, Letter, Email and Web Portal applications. Our care management correspondence,
documentation and letter generation functionality is imbedded within Click4Care
enabling a streamlined approach for the creation of accurate, comprehensive internally-
and externally-oriented documentation with template-based forms and letters, all
integrated with configurable Provider and Member Portals, allowing everyone within
the care continuum to collaborate on the Member’s plan of care in real-time.
- Configuration - Click4Care delivers the ultimate care management toolbox filled
with tools for dynamically configuring and automating any and all aspects of your
care management business (you can change configuration and automations in real time
as often as you like). All of our configuration and authoring tools are GUI based
tools that allow your subject matter experts the power to mold Click4Care to your
unique workflows, business processes and policies instead of relying on tight IT
resources or the vendor.
- Care Plan - Within Click4Care Foundation is a full suite of content authoring and
workflow management tools, allowing our customers to define, copy/edit, and update
assessments, including business rules that align program-specific materials, interventions,
talking points and correspondence to the appropriate health and wellness, case,
or disease management activity. This, along with our Care Management Pathway’s content,
provide the ultimate in flexibility for you medical policy.
- Clinical Content - With Click4Care you get the best of both worlds- Click4Care provides
a highly configurable care management platform that allows our customers to leverage
their own clinical content, or defer to Click4Care developed content extensively
researched and incorporating evidenced based guidelines as well as nationally accepted
guidelines from organizations such as the CMSA, DMAA, NCQA, URAC, National Guidelines
Clearinghouse, and other specialty organizations. Our Care Management Pathway’s,
pre-defined assessments and care plans are based on current industry best practices,
compliant with NCQA/URAC standards. We also have out of the box integration to InterQual
and Milliman clinical criteria.
